By submitting my photograph, I hereby give my consent for all purposes, for the reproduction, publication, or other use of photographs of me in all forms, any media and manner in which I may be included in whole or part, with or without my name. By giving my consent, I release Cass Creek International, LLC, their vendors, and assignees from any liability arising from this use, publication, or sale.

I waive any right to inspect and/or approve the final product, copy or script, that may be used in connection with these images. I also release and discharge Cass Creek International, LLC, their vendors, and assignees and all those acting under the photographer's permission of authority, from any liability arising from any alteration, optical illusion, distortion, blurring, or composite use, intentional or otherwise, that may occur or be produced while taking the photographs or during the production of the final product.
